Insurance Information

We are happy to process any insurance claim as a service to you at no charge. Please keep in mind dental insurance does not usually cover your total bill.  Any insurance estimates are only estimates, and you are responsible for all fees in their entirety.

If you have dental insurance, we will file a claim for you.  Any estimated co-pay will be due in full at the first treatment visit.  You can pay the co-copay with cash, check, or credit card.

Any exam or evaluation fees are to be paid in full at the time of the appointment with cash, check, or credit card.  If you have dental insurance, we will file a claim for this evaluation.

PLEASE NOTE:  Your insurance is a contract between you, your employer, and your insurance company.  This office is not a party to that contract.  While the filing of your insurance is a courtesy we gladly extend to you, all charges are your responsibility.

To better assist you with your insurance needs, please bring all information for your dental insurance plan to your evaluation appointment (such as subscriber's date of birth, ID, social security number, etc.).
