    First Visit

Your initial appointment will consist of a evaluation explaining your diagnosis and treatment options. 

Please assist us by providing the following information at the time of your evaluation:

• Your referral slip from your referring dentist and any x-rays you may have.
• A list of medications you are presently taking.
• If you have dental insurance, please bring the information for your policy. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.); or if you are on any medication (i.e. heart medications, aspirin, anticoagulant (blood thinner) therapy, etc.); or if you require medication prior to dental appointments (such as antibiotics for pre-medication).

Dr. Morgan and his staff take GREAT pride in having an immaculately CLEAN office.  From the reception area, to the restrooms, to the treatment rooms, our entire office is kept meticulously clean so you always feel safe and comfortable during your visit.
